You can choose to apply your in-office virtual certificate toward a future visit or request a refund. Simply let us know your preference by completing the form below.

Once we receive your request, a member of our team will verify your information and process it accordingly.

If you choose to apply your in-office virtual certificate to your account for a future visit, it will be available within 3 business days.

If you request a refund, a check will be mailed to the address you provide. Please allow 4-6 weeks for delivery after your request has been verified and processed.
